In suspected myxedema coma, give stress-dose glucocorticoids (e.g., hydrocortisone) before or with thyroid hormone until concomitant adrenal insufficiency has been excluded. Thyroid hormone increases cortisol requirements/clearance and can precipitate adrenal crisis in a patient with unrecognized adrenal insufficiency.
