In a patient with autoimmune disease and immunosuppression, rapidly progressive kidney injury with tubulointerstitial nephritis and systemic findings (e.g., rash) should prompt consideration of drug-induced disease such as DRESS, while ruling out infections and hematologic malignancy; kidney biopsy can establish the diagnosis.
