Diarrhea can be inflammatory or non-inflammatory. Inflammatory diarrhea is associated with rectal bleeding (indicative of structural breach) and systemic symptoms. HLH can present with an inflammatory picture with pancytopenia and splenomegaly, and can be triggered by infections and by medications like checkpoint inhibitors.
