FALLS: what was the environment, what was the predisposing factor? What systems could be involved? Vision, sensory, motor, CV Alarm feature: Insight into the fall UNILATERAL NEURO: Intrinsic reaction to evaluate contralateral CNS. PAIN points away from the CNS and towards the nerve/nerve root. NO MISS: Acute Limb Ischemia (Patient has embolic risk factors) JOINT PAIN: Associated symptoms of weakness and numbness consider outside of joint involvement (is this gout? Nerve involvement?) NEURO EXAM: Pain, Anterior Thigh, Patellar Reflex, Knee Extension points towards L2-4 nerve roots/plexus. Pathology: Is this ischemia or compression? Imaging could help differentiate. Patient has risk factors for bleeding on AC and embolic etiology given AFib. Reflections: Don’t forget to evaluate for ischemia with acute unilateral neurological symptoms Lumbar Plexopathy: Anterior thigh sensory loss, anterior groin pain, hip flexion weakness, absent patellar reflex Weakness + Pain: Why is the weakness so profound but the sensory stimulus of pain is getting through? Think about disease in the limb
