Acute Cough: viral infection >>> other (base rate) Subacute cough -> broaden the ddx Understanding the Background: Medications associated with Asthma, and hx of hemoptysis. Could this be an asthma mimic? Vascular, Infection, Immunodeficiency (congenital) Localization in the Lung: Airway v Alveoli Alveoli (image +): ex) ILD Airway (PFT +): ex) Asthma Respiratory symptoms + normal O2: Less likely alveoli and more likely airway (hypoventilation vs hypoxia) PAN-Pulmonary: lower and upper based on exam Hx of Hemoptysis: Is this pulmonary only or other organ involvement? Look for involvement of small vessels, kidneys, hematology Bronchiectasis: Fundamental Immune Issue w/ Airway Destruction: Too much immune system: Eosinophilic Bronchiectasis Too little immune system: Focal immune deficiency (non-tuberculous mycobacteria). Structural deficiency vs immune deficiency (immunoglobulins vs ciliary function) Fluid (CF) or Ciliary problem: Important now with different treatments Do NOT rely on Situs Inversus to dx PCD (occurs in less than 50%) Use Cl sweat testing, imaging pattern of bronchiectasis, EM microscopy or ciliary testing/genetics
