Do not miss STEMI in patients with chest pain, tachycardia, hypotension, or heart-failure features. Obtain an ECG immediately; inferior STEMI shows ST elevation in II, III, and aVF with reciprocal changes in I and aVL. ST elevation in aVR suggests severe left-main or multivessel disease. Activate the cath lab urgently.
