When evaluating edema, first determine whether it is transudative or exudative, then localize the mechanism. Bilateral edema usually reflects a systemic transudative process (heart, liver, kidneys), whereas nephrotic syndrome causes non-structural hypoalbuminemic edema and nephritic syndrome causes systemic inflammatory disease with hematuria/anemia.
