Ask the time course first; three weeks is subacute. Convulsive movements with altered mental status are not always seizures. Localize: right-sided motor signs point to the left motor cortex. Label this meningoencephalitis, then consider abscess, tumefactive demyelination and vascular causes. Poor gray-white differentiation on CT suggests edema.
