In a “nonfocal” picture of abdominal pain, headache, and neck stiffness with bland CSF and a negative CT, do the pelvic exam, because cervical motion tenderness is enough to diagnose and treat PID even with negative chlamydia/gonorrhea PCR, and a post-LP CSF leak can then muddy the headache story.
